Sacrament Certificate Request
- Any member of the Christian faithful has a right to obtain a certificate of a Sacrament he or she has received and which was recorded in a sacramental register. Only that person, the parents of a minor, someone with legal guardianship of another, a bona fide pastoral minister at another Catholic parish, or a diocesan tribunal official have this right.
  
  All requests for Sacrament certificates must be submitted in writing. Telephone requests will not be accepted. Please use the form below to request a Sacrament certificate.
  
  We are only able to mail Sacrament certificates directly to a Catholic Parish or Catholic agency on your behalf. Otherwise, Sacrament certificates must be picked up by the person requesting the certificate. A valid photo identification will be required at the time of pick up.
